Whether you are Social Work Funded, Direct Payments, ILF or Self Funding, we are here to assist you with all your care/supported living needs.
In order to ensure we are providing you with a person centred service we will:
• Visit you to discuss all your needs (including risk assessment)
• Agree person centred care plan Identify care/supported living team
• Identify and fulfill any further training needs
• Deliver agreed care/supported living package
• Meet regularly to review your service and amend where needed
This will ensure we give you the high standard of service you deserve whatever your care/supported living needs.
